Request Activation of Target of Opportunity

  THE PRINCIPAL INVESTIGATOR (PI) or a suitable representative must be available 24 hours a day, preferably by phone, so that the proposal can be properly implemented and scheduled. After submitting this form, the PI MUST contact someone at the institute by phone (list provided at the end of this form) to verify that we have received your request.
     Activating Your Target of Opportunity Proposal describes the steps taken to schedule your observations.
 
To request activation of your ToO observations, please provide us with the following information:

Reason for Activation Please describe what has happened to justify activating this proposal (e.g. a nova outburst, a supernovae, a gamma ray burst, a newly discovered comet, and so on) and why the observations need to be done now. The reason should be consistent with the type of event and plan of observations already specified in your proposal.

If you answered yes, please specify them. (Note: These changes may require resubmission of your Phase II proposal)
Scheduling Requirements For each visit please specify the earliest and latest start times to meet your program's requirements. Be as flexible as you can. If we cannot accomodate these windows due to HST restraints, your request may be declined.
